In the Old Man Logan storyline, The Hulk had inbred grandchildren who enjoyed killing people, so it would be safe to assume he was dead there, but they never said how.
In one of the realities visited by the Exiles, the Hulk was killed by a setting off a concentrated gamma bomb a few feet away from him. Of course, this created an even stronger Hulk in Simon Williams (Wonder Man), who was standing a little further away from the blast.
- My guess is that the Muramasa blade could do the job. Since it is powerful enough to negate superhuman healing properties (due to it having a shard of Wolverine's soul inside of it), if someone managed to severely damage Hulk with it, he'd be down for the count for good. Sabretooth was killed that way. That is, assuming that no one can snuff Bruce Banner with one clean shot without giving him a chance of going Hulk (which I don't quite find possible, given the circumstances).
