I think you asked How does the conduction of heat takes place. Practical example. Heat 1 end of a metal rod and after sometime you are able to feel the heat on the other end. This is because heating excites the metal rod molecules which are in a constant state of flux. The excited molecules transverse its energy to its neighbour molecules and the neighbours to their neighbours until finally it reaches to the molecules near your hand and hence you perceive this as heat.
