Friar Lawrence is to blame for Romeo and Juliet's death for many reasons.

He is the one who married Romeo and Juliet. And so if it wasn't for him, they wouldn't have died for one another.

Another reason is that he gave Juliet the sleeping potion/drug that caused her to appear dead. If he hadn't given it to her, then Romeo would not have known that she had "died".
